Doxy-PEP in Thailand: How It Works and Who It Is For

Doxy-PEP in Thailand explained: the 200 mg dose, who guidelines suggest it for, side effects in the tropical sun and why it never replaces HIV prevention.

01 — The short answer

An antibiotic after sex, for some bacterial STIs.

Doxy-PEP is a single 200 mg dose of the antibiotic doxycycline, taken within 72 hours of condomless sex, to lower the chance of syphilis and chlamydia, and to a lesser extent gonorrhoea.

If you are searching for doxy PEP in Thailand, the key point is that it is a doctor’s decision, not a holiday supply. It suits some people with a pattern of repeat bacterial infections, and it does nothing at all against HIV, herpes, HPV or hepatitis. It works best as one layer alongside testing, condoms and HIV prevention.

We don’t list doxy-PEP as a routine service, so ask us whether it suits you. If the sex happened in the last 72 hours, HIV comes first: read about the 72-hour PEP window.

02 — How it works

One dose, ideally the next morning.

The usual DoxyPEP dosage is 200 mg of doxycycline, taken once, ideally within 24 hours and no later than 72 hours after sex.

CDC guidance adds a ceiling: no more than 200 mg in any 24-hour period, however many partners there were that day. The antibiotic sits in the body long enough to stop bacteria picking up a foothold before an infection settles. It is not a daily pill and it is not taken before sex.

03 — Who it is for

Guidelines name specific groups.

Current guidance aims doxy-PEP at people whose risk of bacterial STIs is clearly raised, not at everyone who has sex.

The 2024 CDC guideline suggests discussing it with gay, bisexual and other men who have sex with men, and trans women, who have had at least one bacterial STI in the past year. The 2025 UK national guideline focuses on preventing syphilis and names cisgender gay and bisexual men and trans women as the main group, while allowing others at increased risk of syphilis.

For cisgender women the picture is different. A Kenyan trial in 449 women found no significant drop in chlamydia or gonorrhoea, and researchers are still working out why. Women, trans men and non-binary people should get individual advice rather than follow the headlines.

Wherever you are, a prescription should rest on a doctor’s assessment of your own situation, not on a general rule.

04 — Where it fits

Doxy-PEP vs PrEP: different jobs.

The names sound alike, but doxy-PEP targets bacteria and PEP and PrEP target HIV, so none replaces another.

This is the most common mix-up we hear. Someone on doxy-PEP still needs HIV protection, and someone on PrEP still needs STI checks. Our PEP vs PrEP comparison covers the HIV side in depth, and what HIV PEP is explains the emergency medicine.

Many people who ask about doxy-PEP already take PrEP in Pattaya, which makes a combined review simple.

05 — The debate

Gonorrhoea and antibiotic resistance.

Doxy-PEP is weakest against gonorrhoea, and the reason matters for anyone having sex in Asia.

Many gonorrhoea strains already shrug off tetracyclines, the family doxycycline belongs to. The BASHH guideline says doxy-PEP is unlikely to prevent gonorrhoea in the UK because resistance there is above 90%. Tetracycline-resistant gonorrhoea is widespread in other regions too, so don’t count on doxy-PEP for gonorrhoea during a Pattaya trip.

Specialists also watch the wider effects. Studies have seen more tetracycline-resistant gonorrhoea and staph bacteria among users, and doxycycline can shift the gut’s normal bacteria. Syphilis and chlamydia show little resistance so far, which is why guidance keeps doxy-PEP targeted and reviewed rather than open to all.

You can read the reasoning in the BASHH 2025 doxyPEP guideline.

06 — Side effects

Easy to take, but mind the sun.

Most people have no trouble with doxy-PEP, but the common side effects are stomach upset, a sore gullet and skin that burns easily.

That last one matters in Pattaya, where the midday sun is strong all year. Doxycycline can make skin burn far faster than usual, with redness or a rash on areas that would normally tan. A day on Jomtien beach or a boat trip to Koh Larn is exactly when it shows up.

Doxy-PEP side effects and how to reduce them

Side effectWhat it feels likeHow to reduce it
Sun sensitivityFast sunburn, rashHigh-factor sunscreen, shade at midday, a hat
NauseaQueasy stomachTake it with food
Throat irritationBurning behind the breastboneFull glass of water; stay upright for 30 minutes
Less absorptionNo symptomsKeep antacids, iron and calcium a few hours apart

Doxycycline is usually avoided in pregnancy. Tell the doctor about other medicines, allergies or any chance you are pregnant.

07 — Testing

Testing still sets the rhythm.

Doxy-PEP lowers the odds of an infection; it does not tell you whether you have one, so regular checks stay essential.

CDC guidance advises STI testing at every site you use for sex before starting, then every three to six months, with the need for doxy-PEP reviewed at the same visits. That rhythm catches infections that slip through and picks up gonorrhoea early. Our guide to STI testing in Pattaya covers which samples and when.

The same visit is a good moment for an HIV test if you are not living with HIV. HIV testing in Pattaya explains the options and window periods.

08 — In Pattaya or Bangkok

Getting advice, not a pharmacy guess.

Searches for DoxyPEP in Pattaya or DoxyPEP in Bangkok often end at a pharmacy counter, but a consultation is the safer route.

Doxycycline should be prescribed after a doctor has checked your history, recent tests and other medicines. Buying antibiotics to dose yourself risks the wrong drug, an untreated infection and more resistance. The same logic applies to HIV medicines; see can you buy PEP at a pharmacy.

  • Already prescribed doxy-PEP at home? Bring the prescription and your latest results, and keep to the plan your clinic set.
  • New to it? Book an STI check and ask us whether it suits you.
  • Recent exposure? Ask about HIV PEP first, within 72 hours.

Questions

Doxy-PEP, quick answers.

Written by the HIV PEP Pattaya team, based on BASHH and CDC guidance. Last updated 29 September 2026.

What is doxy-PEP?

It is a single dose of the antibiotic doxycycline taken after condomless sex to reduce the chance of syphilis, chlamydia and some gonorrhoea. It is prescribed for people at higher risk of these infections.

What is the doxy-PEP dosage?

Guidelines use 200 mg once, ideally within 24 hours and no later than 72 hours after sex. No more than 200 mg is taken in any 24 hours.

How effective is doxy-PEP?

Trials in gay and bisexual men and trans women showed large drops in syphilis and chlamydia and a smaller one in gonorrhoea. Evidence in cisgender women has not shown the same benefit.

Does doxy-PEP protect against HIV?

No. It is an antibiotic and has no effect on HIV. After a possible HIV exposure you need HIV PEP within 72 hours, or PrEP for ongoing protection.

Can I buy doxycycline at a pharmacy in Thailand?

Self-dosing with antibiotics bought over the counter is not advised. A doctor should check whether doxy-PEP suits you, arrange STI tests and plan reviews every three to six months.
